Patient Forms

Exciting News!  Our office has gone (mostly) paperless!  Upon arrival for your appointment, we will help you get checked in using an application on our office iPad.   Once you are checked in, you will be able to easily fill out the forms using this device, plus update any important information we should know about.  You will notice the iPad has easy to choose buttons, a large font and fillable fields using a keyboard or touch screen.   If you would like assistance, simply let one of our administrative team know and someone will be out to help.

Patient communications & appointment Reminders

We understand how hectic schedules can be for our patients and their families, you are running to school, work, sports practices, meetings and more...and who has time to remember when you scheduled that last dental appointment anyway?  Well, no worries, we have a system to help you remember everything - even at the last minute!   Our software uses a combination of text, email, and phone reminders and confirmation communications to help you make sure your appointment is on your calendar.  Here's what you can expect:

• Upon scheduling an appointment, you will receive a Save the Date email and text that includes an "add to calendar" feature!
• 30 days before your appointment, we will send a friendly reminder via postcard, text and email, helping make sure your reserved date still works for you.
• Starting 10 days before your appointment, you will begin receiving communications to allow you to confirm your appointment.  Simply text back the letter C to let us know you can still make it.
• Then, because life gets chaotic, we'll send you a friendly reminder the day before and the morning of your appointment.

Need to change your time/date?  That's easy - simply reply to the text and we'll help you get everything re-arranged.
